Our team brings together expertise in occupational therapy, nursing, aging in place, care management, advocacy, and local resources to help older adults and families understand their options, make informed decisions, and put the right supports in place.
Our services are flexible and personalized. Some families need professional guidance for a short period of time, while others rely on us for ongoing support as circumstances change.
Aging Life Care Management
$175/hour | One-hour minimum
Professional assessment, advocacy, coordination, and guidance when you need someone experienced to help manage the bigger picture.
Aging Life Care Management can begin with an assessment of your current situation and may continue for as long, or as little, as professional involvement is helpful. We tailor our role to the needs of each client and family rather than requiring a predetermined package of services.
Care Management may include:
- Comprehensive assessment of needs, strengths, concerns, and priorities
- Development and implementation of a personalized care plan
- Healthcare advocacy and support with medical appointments
- Coordination with physicians, senior living communities, home care agencies, therapists, and other providers
- Hospitalization, rehabilitation, discharge, and transition support
- Medication and healthcare system navigation
- Home safety, accessibility, and aging-in-place recommendations
- Research and coordination of community resources and services
- Family communication, guidance, and problem-solving
- Ongoing monitoring and reassessment as needs change
- Support during unexpected changes or more complex situations
You only pay for the professional support you need. Some clients engage us for assessment and short-term guidance, while others benefit from an ongoing relationship as health, living situations, or family needs evolve.
Focused, Founder-Led Consultation
Aging Strategy Session with Terri

An Aging Strategy Session is a focused one-on-one virtual consultation with Terri Lemere, Founder of Healthy Home Living Solutions, Occupational Therapist, Certified Aging in Place Specialist, and Aging Life Care Manager.
Bring your questions, concerns, and the situation that’s keeping you wondering, “What should we do next?” Terri will help you sort through the issues, identify priorities, consider your options, and develop practical next steps.
A Strategy Session may be a good fit if:
- You’re beginning to notice changes in a parent or loved one and aren’t sure what they mean
- You’re trying to determine whether more help is needed at home
- Your family is considering senior living or other care options
- You’re navigating a discharge from the hospital or rehabilitation
- You and other family members aren’t sure what should happen next

Your Strategy Session Includes:
- A brief pre-session questionnaire so we can focus on what matters most
- A private 60-minute virtual consultation with Terri
- Focused discussion of your questions, concerns, priorities, and options
- Practical recommendations and next steps
- A concise follow-up summary of the key recommendations discussed during your session
A Strategy Session provides professional perspective, recommendations, and direction so you can decide how you’d like to move forward.
It does not include comprehensive assessment, medical record review, communication with healthcare providers, ongoing research, coordination of services, implementation of recommendations, or ongoing case management.
If your situation requires more active professional involvement, Aging Life Care Management may be the better fit, and we can talk about that during your session.
Personal Assistants for Seniors & Life Enrichment
$60/hour | One-hour minimum per visit
Personalized, non-medical support that helps older adults stay engaged, organized, connected, and supported in everyday life.
Sometimes an older adult needs a little extra support to stay organized, engaged, connected, and confident in everyday life. A trusted Personal Assistant can provide practical assistance, companionship, organization, and meaningful engagement.
Support may include:
- Companionship and meaningful conversation
- Life-enrichment activities based on individual interests and abilities
- Organization of mail, paperwork, calendars, and household tasks
- Technology assistance
- Errands and shopping
- Accompaniment on outings and community activities
- Regular check-ins for older adults living independently
- Support with projects and tasks that have become difficult to manage alone
- Practical follow-through on recommendations established through Care Management
Flexible scheduling without traditional shift minimums. Unlike many traditional caregiving services, our Personal Assistants are not structured around multi-hour shift requirements. Visits can be scheduled based on the individual client’s needs and the availability of the matched Personal Assistant.
Personal Assistants provide non-hands-on support and do not provide personal care or assistance with activities of daily living such as bathing, dressing, toileting, transfers, or feeding. When hands-on care is needed, we can help families identify appropriate caregiving resources.
Personal Assistant services may be used independently or alongside Aging Life Care Management. When services are combined, our Care Managers can oversee the bigger picture while a Personal Assistant provides consistent one-on-one support.
