In order to establish the best action plan for you and your loved one, I can provide a thorough assessment of every aspect of your loved one’s life. Through this process, we can determine which areas need to be addressed and will guide our plan of action, taking into consideration your loved one’s wants and needs. Crisis Intervention
Life happens! Hospital discharges can happen very quickly. Families are often notified of an impending discharge with less than 24 hours notice. In that brief time, they need to make critical decisions about post-hospitalization care. Healthy Home Living Solutions is here to assist and advocate for your loved one, while navigating the care process for you. Our goal is to ensure that you or a loved one has the absolute best care and what might be needed for a full recovery.
Medical Care Coordination
As we get older, new and intimidating conditions may come into our lives. Conditions such as dementia, Parkinson’s, stroke and others can be scary, and put us in a state of “how do I handle this?” With my 26 years experience as an Occupational Therapist, I will help you with understanding diagnoses, treatment options and care needs. We will assess, then educate you on your options, and provide services to ensure your loved one has the right care.
